Sight for Students
There are five basic qualifications your child must meet in order to receive a Sight for Students gift certificate.
- Family income is no more than 200% of federal poverty level (see chart below).
- Child is not enrolled in Medicaid or other vision insurance.
- Child is 18 years old or younger and has not graduated from high school.
- Child or parent is a U.S. citizen or legal immigrant with a social security number.
- Child has not used the Sight for Students program during the past 12 months.
Healthy Eyes Eye Glass Program
- Qualifications that must be met in order to qualify for the Healthy Eyes Eyeglass Program.
a. are at 200% of the current Federal Poverty Guidelines;
b. do not have any other eyeglass benefits they can access;
c. have a valid eyeglass prescription that is less than two years old;
d. have participated in a Prevent Blindness screening program or have been referred by a Prevent Blindness partner agency; and have not previously participated in this program (unless there has been a change in their prescription or their eyeglasses have been damaged beyond repair).
